It's like a neurosis. A hypochondriac knows he isn't sick, it's his way of talking about his emotional state.
Likely he can't admit it to himself, unless under pressure, but he knows. Except for the full on wackos that can't tell the difference between reality and fantasy, they know.
So he knows, most of the time.
